Participation Agreement, Liability Waiver & Program Policies

All LVA Programs  |  Effective upon completion of registration


Scope of This Agreement

This Agreement applies to all Luna Volleyball Academy (LVA) programs, clinics, events, and training sessions — current and future — in which the participant enrolls. A complete list of current program offerings is available at lunavolleyballacademy.com.


I. Acknowledgment of Risk

I acknowledge that I understand the nature of volleyball, jump training, and all other sports and athletic activities offered by Luna Volleyball Academy. I consent to my child's — or my own — participation in such activities and confirm that the participant is in good health and physical condition.

I fully understand that the risks of participating in sports and athletic training activities may include, but are not limited to, common athletic injuries such as sprained ankles, tendinitis, muscle strains, and joint injuries. These risks are inherent to the sport and are not the direct result of LVA's instruction or activities.

Emergency Authorization: I authorize any LVA coach, instructor, or trainer to act on my behalf and use their best judgment in any emergency requiring immediate medical attention. I agree to be responsible for all medical costs incurred.


II. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue Luna Volleyball Academy, its owners, directors, coaches, instructors, staff, volunteers, and affiliated entities (collectively "LVA") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by the participant while participating in any LVA program or event, or while on or about LVA-associated premises.

This release includes, without limitation, claims for negligence to the extent permitted by applicable law. I acknowledge that this release has been freely and voluntarily entered into with full knowledge of its contents.


III. Code of Conduct

LVA is committed to providing a safe, respectful, and positive training environment for all participants. All athletes, parents, and guardians are expected to:

  • Treat coaches, staff, fellow athletes, and facility personnel with respect at all times.
  • Arrive on time and prepared to train (appropriate athletic attire and footwear required).
  • Refrain from the use of profane, abusive, or discriminatory language.
  • Follow all instructions given by LVA coaches and staff.
  • Refrain from using mobile devices during training sessions unless directed by staff.
  • Respect the training facility, equipment, and the property of others.

Dismissal Policy: Behavior deemed egregiously disruptive, disrespectful, or inappropriate — in the sole judgment of LVA instructors — may result in immediate dismissal from training without a refund. LVA reserves the right to deny future enrollment to any individual who violates this policy.


IV. Attendance & Make-Up Policy

  • 24-hour notice required: Please provide LVA at least 24 hours advance notice for any session cancellation or change request. Requests given with less than 24 hours' notice are not eligible for credit or make-up.
  • Session credits: A credit for another session may be issued if LVA is notified at least 24 hours in advance. Credits must be used within 30 days and will expire without refund.
  • No refund for missed sessions: Sessions not cancelled per the 24-hour policy are forfeited and are not eligible for credit or refund.
  • Injury during program: No refund is issued for injury sustained during training. See the Medical Refund section below for applicable procedures.
