All-Access Summer Training Pass

I. ACKNOWLEDGE OF RISK

I acknowledge that I understand the nature of volleyball, jump training, and all other sports and athletic related activities. Therefore, I consent to participate in such activity in good health and physical condition.

II. I fully understand that the risks of participating in sports and athletic training activities may include common athletic injuries, such as sprained ankle and tendinitis, and are not a direct cause of sports and athletic training instruction and activity.

III. I authorize any LVA instructors or trainers to act on my behalf according to their best judgment in any emergency requiring medical attention. I release Luna Volleyball Academy coaches and any associated instructor of all liability.

IV. I acknowledge that I will receive no refund for injury or during training.

V. I acknowledge that the behavior by my child or myself deemed egregiously disruptive, disrespectful, or inappropriate in the judgment of the instructors may result in dismissal from training without a refund.

VI. PHOTO RELEASE

I unconditionally authorize LVA to take audio recordings, photographs, and video footage of my child or me during all training programs I participate in. I give Luna Volleyball Academy permission to use my child’s image and my own for all purposes related to Luna Volleyball Academy, including but not limited to promotional materials for training programs, video tutorials, instructional videos, blog posts, YouTube channels, etc. I release all rights to the photographs and/or video footage taken during all the Luna Volleyball Academy training programs without expecting any monetary compensation or goods and services in return.

VIII. CANCELATION/REFUND POLICY

I will give LVA a 24-hour notice before canceling and postponing a training session agreed upon in advance. I acknowledge that I will not receive a refund if cancellation postponement is given under 24 hours notice. I acknowledge that I must notify LVA at least 24 hours prior for all change requests to be considered. I accept that I may receive a credit for another program if I notify LVA at least 24 hours before the training I signed for. I agree that If I don’t use the credit within one month of receiving it, the credit will expire, and I will NOT issue a refund. 

I acknowledge that LVA will only process a request for a refund If my child is medically unable to participate in the program she had registered for and that I must show a doctor’s explanation with a precise diagnostic of the injury, and that letter must be with a letterhead clearly stating that my child cannot participate in the program signed.

I acknowledge that the amount refunded is relative to how much of the season or camp has passed.
